How about pulling a cord with thorns through your tongue?
Check out the Blood of Kings by Schele and Miller, 1986, in
which one can find out about the spectacles of the Mayan royalty. The
cord routine by female royalty can be found illustrated in the relief
carvings here at Palenque. The men used a sting ray spine and ran it
through their now not so private parts with a paper fan attached to it
which dropped into a basket. This turned bright red, to the delight
and amusement of the throngs of the onlookers gathered in front of the
structures where the ritual was performed. Blood was an essential
part of the Mayan religion. After the Spanish made their conquest,
they were aghast at this pagan horror, so they burned its followers at
the stake and brought bull fighting to the New World, clearly an
improvement.
